About this deal

Secondary care use only as a mouthwash for a variety of oral mucosal diseases.Also available for use by ENT surgery. Benzydamine: Difflam Spray - Each metered dose pump spray delivers Benzydamine hydrochoride 0.15% w/v, approximately 175 microlitres per spray. Contains methyl parahydroxybenzoate and Ethanol Designated by NHS England to require initiation by or in prior agreement with a Specialist Centre. Continuation where appropriate by hospital (or other secondary care provider)

Folinic acid Mouth wash: May be useful in cases of mucositis following Methotrexate chemotherapy (limited evidence base) Pour the entire contents of the single-dose Gelclair sachet into a glass and add approximately 40 ml of water (3 tablespoonfuls). Stir mixture well and use at once. Rinse around the mouth for at least one minute or as long as possible to coat tongue, palate, throat, inside of cheeks and all oral tissue thoroughly. Gargle and spit out. Antihistamine nasal sprays tend to be less effective than topical steroids. They offer an alternative for a small number of patients (or parents) that express concern about the long-term safety of corticosteroids. Local infection should be treated.

Fluconazole should only be used to treat proven fungal infections. Fluconazole should NOT be used prophylactically except in Neuroblastoma high risk patients.Benzydamine oral rinse (‘Difflam’) is useful where there is painful inflammation of the mouth and throat. The recommended dose is 10-15mL used every 1.5-3 hours. Dilute with an equal volume of water if stinging or numbness occurs. Mouth care is an essential aspect of palliative care in all settings and should be considered part of daily routine patient care, see the Scottish Palliative Care Guidelines, Mouth Care.

There is no convincing evidence that antiseptic lozenges and sprays have a beneficial action. Sprays that contain local anaesthetics may cause sensitisation. SFH information: Replacement for BioXtra. If using part of a MST sachet mix well with water and use immediately. MST sachets should not be used for doses <5mg as sachets do not disperse equally therefore dosing may be inaccurate Due to the anaesthetizing effect on the pharynx, difflam should not be used for pre-school children or immediately before meals as it could lead to choking. Care should be taken with very hot or cold drinks. The patient’s mouth should be regularly examined by a nurse/doctor/dentist using the otoscope/torch as part of routine assessment to observe any changes ie inflammation, ulcerated areas and candida plaques using the scoring chart below ( see Appendix 1). External changes ie sore or vesicles on the lips should also be noted and bacterial/viral swabs taken where necessary. Use of additional aids eg floss, fluoride tablets and electric toothbrushes – by recommendation of the dental team only.
